Chemical Use Assessment

Information Sheet (169A.70, 254A.03)

The Court has ordered that you complete a Chemical Use Assessment and that the assessment be provided to the Probation Department/Court. A Chemical Use Assessment provides the Court information that identifies if there is a problem associated with your use of chemicals and what level of care could be recommended to address the chemical use. The Court ordered Chemical Use Assessment can be completed one of three ways:

  1. Private Provider – If you choose to have your CUA done by a private provider in the community, you will be provided a list of providers in our area. Private providers all charge different rates for an assessment and take traditional forms of payments as well as insurance to help cover the cost. It is your responsibility to let the provider know you have been referred by the Court along with the name of your assigned probation officer. It is also your responsibility to provide your completed assessment to the Probation Department/Court.
  2. Probation Department – Chemical Use Assessment Screenings are also completed by the Probation Department. This department charges $100.00 which is a similar rate as the private providers in the community, but we do not accept private insurance. When a CUA screening is done by the Probation Department and it is determined you may need to be referred on for additional CUA services, you will need to go to a private provider in the community for that service. For example, you may be referred to a more in depth Chemical Use Assessment or treatment as determined.
  3. RULE 25 Assessments – Chemical Use Assessments can also be provided by Isanti County Family Services if you meet the minimum financial criteria where you are not able to pay for an assessment, are requesting treatment services, and have no other insurance that would pay for the assessment.

It is your responsibility to contact our department for a list of providers if you are choosing a private provider for your CUA or to schedule a CUA screening by our department. The Chemical Use Assessment must be completed by the date as ordered by the Court or no later than three weeks after the date you were ordered by the Court.

Isanti Probation Department does not endorse or ensure quality of service from any particular source. We are aware that these entities have provided chemical use assessments on a fee basis. If you have insurance, we recommend you approach your insurance company to seewhat, if any, coverage your insurance plan may have for chemical use assessment and/or treatment.

 

Presentence Investigation

The Court has ordered that you complete a Presentence Investigation and that the results be provided to the Court. A scheduled appoinment with an assigned Probation Agent will be required for you to complete the Presentence Investigation.
